To schedule your grocery shopping trip, you will need a photo ID, proof of residency in Covington, proof of income (latest W2, recent paystubs or other proof of income) and a brief conversation to determine size of household and current circumstances. Please print off and fill out our short Intake Survey below, then call us to schedule a time to drop off your paperwork to our Program Coordinator.   Click here for our Meijer Guidelines.

For payment of utility or rent, please provide all information listed above, along with a photo of current statement with your name and address showing. Rent payments, please provide contact information for landlord including name, address, phone number, and email if possible.  Please call us to arrange a time to drop off your paperwork to our Program Coordinator.
